**Responsibilities:**

Conduct patient assessments and develop individualized treatment plans.

Provide hands-on therapy and rehabilitation services in outpatient and home settings.

Monitor patient progress and adjust treatment plans as necessary.

Educate patients and families on exercises and health management.

**Qualifications:**

Licensed Physical Therapist in Illinois.

Strong communication and interpersonal skills.

Experience in outpatient or home health settings preferred.
