We are seeking a dedicated Home Care Physical Therapist for a short-term contract to provide in-home therapy services. The ideal candidate will have a passion for improving patients’ quality of life through personalized care.

Responsibilities:

• Conduct comprehensive assessments of patients’ physical abilities.
• Develop and implement individualized treatment plans.
• Educate patients and families on exercises and mobility techniques.
• Document patient progress and adjust care plans as needed.

Qualifications:

• Licensed Physical Therapist in NY.
• Experience in home care preferred.
• Strong communication and interpersonal skills.
