We are seeking a dedicated Home Health Physical Therapist (PT) to join our team in Sonoma, CA. This role offers the opportunity to provide personalized care to patients in their homes, enhancing their mobility and quality of life.
Responsibilities:
Conduct patient assessments and develop individualized treatment plans.
Implement therapeutic exercises and modalities to improve patient outcomes.
Educate patients and families on home exercise programs and safety.
Document patient progress and adjust treatment plans as necessary.
Requirements:
Valid California PT license.
Experience in home health or outpatient settings preferred.
Strong communication and interpersonal skills.
Salary: $60 – $75 per hour.
