Job Title: Physical Therapist – Home Health

We are seeking a dedicated Physical Therapist to join our home health team in Rhode Island. This role offers the opportunity to provide personalized care to patients in the comfort of their homes, enhancing their quality of life.

Responsibilities:

• Conduct patient assessments and develop individualized treatment plans.
• Implement therapeutic exercises and modalities to improve mobility.
• Educate patients and families on rehabilitation techniques.
• Document patient progress and adjust treatment plans as needed.

Requirements:

• Valid Physical Therapy license in Rhode Island.
• Experience in home health preferred.
• Strong communication and interpersonal skills.
• Ability to work independently.

Salary: $60 – $125 per hour
