Job Title: Physical Therapist Home Care PRN Location: Grand Rapids, MI Salary: $67 – $69 per hour

We are seeking a dedicated Physical Therapist to join our home care team on a PRN basis. The ideal candidate will provide high-quality, patient-centered care to individuals in their homes.

  • Conduct comprehensive assessments and develop personalized treatment plans.
  • Implement therapeutic interventions to improve mobility and function.
  • Educate patients and families on rehabilitation techniques and home exercises.
  • Collaborate with healthcare professionals to ensure optimal patient outcomes.

    • Qualifications: Valid PT license in MI, strong communication skills, and a passion for patient care.
