The ideal candidate will provide exceptional patient care, develop treatment plans, and collaborate with healthcare professionals to ensure optimal patient outcomes. Responsibilities:

• Conduct patient assessments and evaluations.

• Develop and implement individualized treatment plans.

• Coordinate care with multidisciplinary teams.

• Educate patients and families on rehabilitation processes.

• Maintain accurate patient records and documentation.

Qualifications:

Current NV PT license

Strong communication and interpersonal skills

Experience in care coordination preferred
