We are seeking a dedicated Physical Therapist (PT) to join our home health team in San Francisco. The ideal candidate will provide high-quality, patient-centered care in a home setting.

Conduct comprehensive assessments and develop individualized treatment plans.

Implement therapeutic interventions to improve mobility and function.

Educate patients and families on rehabilitation techniques and home exercises.

Collaborate with healthcare professionals to ensure optimal patient outcomes.

Qualifications: Valid PT license in CA, strong communication skills, and experience in home health preferred.
